Dear Mozart Elementary Parents,
 
The following forms on this page are for medication administration at school, and emergency action/care plan for seizures, asthma, diabetes, food allergies and anaphylaxis.  
 
In the event that your child needs to have a prescribed medication administered during school hours, a Parent Request for Administration of Medication at School must be completed and signed by the child's guardian for each medication to be administered during school hours and must be given to the school nurse before the medication can be administered to the child during school hours.
 
Furthermore, if your child has asthma, seizures, diabetes, food allergies or anaphylaxis, a corresponding emergency action/care plan must be filled out by your child's healthcare provider and brought to the nurse's office by the first day of classes or as soon as it is made known to the parent/guardian that the child requires an emergency action/care plan. Each emergency action/care plan must be renewed every school year.
